A 1996 Honda Accord, was proceeding north on Sepulveda Boulevard turning left onto Manchester Avenue when it was struck on the passenger side by a southbound 2000 GMC Jimmy truck. The Honda was pushed toward the southwest corner of the intersection where it struck a 1992 Mazda pickup truck, which was waiting to turn right onto southbound Sepulveda Boulevard from eastbound Manchester Avenue.

After the collision, the driver of the GMC Jimmy, described as a 30 to 35 year old white male, 5′ 10" tall with medium blond hair (long enough to put in a pony tail), and his passenger, described as a 20 to 25 year old white female with long black hair, were seen attempting to blend in with the crowd that had gathered at the scene, then ultimately walking from the scene, southbound on Sepulveda Boulevard. As they left the scene, witnesses noted that the female was walking with a limp. They did not identify themselves or attempt to render aid to the injured persons prior to leaving the scene.
